Records Request

Patients: If you're interested in receiving your medical or fire report, please complete the Records Request Form below. You will need to include a scanned copy of your photo ID which you can upload in the form. Requests cannot be processed without a copy of a valid photo ID. Patients may request their own records free of charge. 

Insurance and Law Offices: A $15.00 processing fee is required in order to fulfill your request.  If a subpoena is required-- Please mail to PO Box 2768 Truckee, CA 96160 or deliver to 10049 Donner Pass Rd. Truckee, CA 96161. Payment options are available within the form. Please note, records can only be released once payment is received.

Investigation Reports: A $40.00 processing fee is required in order to fulfill your request. Make all checks payable to Truckee Fire Protection District.  If a subpoena is required-- Please mail to PO Box 2768 Truckee, CA 96160 or deliver to 10049 Donner Pass Rd. Truckee, CA 96161.
